United Way

Due to the increasing number of adverse drug events and identifying falls as the number one cause of injury hospitalization in their senior population, United Way of Dane County, WI saw a need to provide medication management for these individuals.

The initiative, working together with the Pharmacy Society of Wisconsin (PSW), desired a clinically based, technology platform allowing pharmacists to conduct medication therapy management and enabling the management of the program through standardization and outcomes. 

The platform needed to be efficient: as the longer it takes to do a consult - the less likely pharmacists will opt-in to participate as their schedules are demanding and often unpredictable; and as the fee for service metrics become less advantageous the longer the consults take. The platform also needed to be highly intuitive, as pharmacy teams did not have the time to learn a complicated new system, and clinically best-in-class, in order to ensure premium patient care and outcomes. Finally, not only did the platform need to be scalable, easy to deploy, and cost effective, but it also had to be flexible to accommodate customized program-specific features.

PSW, knowing the MTM solutions marketplace options well, went to Aprexis to provide the infrastructure for the United Way’s Safe & Healthy Aging Program. Aprexis was excited to be a key contributor to this important program. The program launched in September 2011 and is going strong.
